Purpose
When HR receives a National Medical Support Notice (NMSN) it is a federal notice to enroll an employee onto our health insurance IF THEY QUALIFY

When HR receives a National Medical Support Notice (NMSN) it is a federal notice to enroll an employee onto our health insurance IF THEY QUALIFY. To qualify the employee must meet four criteria:

Make enough to afford the insurance.

Be currently employed.
-
Worked with YES for 5 months with no more than a 1-month gap.
-
Worked at least 630 hours during that time.
The Employer Response From will be the form we use to respond to the issuing state agency, regardless of if they are working or not. A Utah Office of Recovery Services will be used for the example.

If you marked 1, 3 or 4, and the employee is no longer working or just started working, complete the bottom section with Employer Name, Rep, phone and fax #s, and fax this page and the first page back to the issuing state agency.
If the employee meets all the insurance qualifying criteria, give the full NMSN packet to whomever is over insurance.
