Procedure Steps
What to do when an injury is reported to you:
-
If possible, request that the client send an e-mail/incident report/supervisor statement as a follow up for documentation purposes or fill out the YES first report of injury located at YES Shared > HR > SAFETY > ~Accident Report~
-
Collect the following information and confirm the best phone # to contact them back at:
-
Who it happened to
-
Where it happened
-
How it happened
-
What happened
-
What time it happened
-
Severity of the injuries
How serious is the injury?
-
Very serious
-
If the injury is life threatening or life or limb, call an ambulance or rush them to the nearest Emergency Room
-
DO NOT LET A SERIOUSLY INJURED EMPLOYEE DRIVE THEMSELF TO THE E.R. OR CLINIC
-
Moderately Serious during clinic hours
-
Find the nearest clinic on the Clinic Priority List and view their hours of operation
-
Head injuries can be very serious! If the employee thinks they are fine, keep them on the premises for at least an hour and allow them to continue to work. If they get dizzy, pass out, faint or throw up then take them to the nearest clinic or hospital
-
Moderately Serious after clinic hours
-
If there isn’t a clinic open nearby and the injury is pretty serious and cannot wait until the morning, take them to the nearest E.R.
-
Not very serious
-
Consult with the employee to see if they can remain on the job with basic first aid treatment, band-aid, ice or over-the-counter medicine (Tylenol, Ibuprofen, Advil, etc). If they choose to go home and forego medical treatment, they must sign a release form. Have a co-worker or family/friend drive them home if they are unable to finish the shift. If they refuse to sign the release form, document the conversation in their Avionte file.
-
Send all injury related information to [email protected]
-
First report of injury and clinic, Workmed, or hospital documentation, etc.
As HR investigates the incident, we may be reaching out to you to help us gather as much information as we can on the incident. This could include helping us get information from the client and from the employee about what happened or reaching out to the client to see if they are able to accommodate the employees doctor recommended restrictions.

RETURN TO WORK PROCESS
It is extremely important that employees do not lose time from work due to an injury. When Worker’s Compensation has to pay lost time, it hits our policy at 100% of the cost for the following 3 years’ worth of policy periods.
We will do our best in EVERY situation to accommodate an employee’s “restrictions” after an accident. We want them back to work the very next day as often as reasonably possible.
